Available Services
We will work with you to create a customized plan of care, and manage it every step of the way. Our experienced team of care management specialists and friendly companions are there to ensure the highest quality of care is consistently provided. Our caregivers schedules are flexible and are available to provide care whenever and wherever it’s needed, while our clients receive the assistance they need to continue to enjoy their regular activities.
Nursing Services
Professional nursing services are provided in accordance with the plan of care and are case managed by licensed Registered Nurses who may also be the visiting nurse assigned. Additionally, case managers may assign LPN’s who service cases under the supervision of the RN and assist in the implementation of the plan of care.
Medical Social Services are provided in accordance with the plan of care and are services by qualified licensed social workers. MSW (and SWA under the MSW) manage the psychosocial needs of patients, assist with financial planning, collaborate with the attending MD to obtain orders, assist with community resources needed by patients, and provide counseling according to the plan of care. Our agency will not prohibit a patient from participating in activities and maintain a schedule that is consistent with the patient’s interest, religious and community activities, assessments and plan of care.
Physical therapy services are provided in accordance with the plan of care and are case managed by qualified, licensed physical therapists who may act as the primary case manager in the absence of the nursing discipline. Additionally, PT case managers may assign PTAs (Physical Therapy Assistants) who service cases under the supervision of the PT and assist in the implementation of the plan of care. PT may develop rehabilitative plans of care, collaborate with the attending MD to obtain orders, provide education to patients and families and set achievable goals to discharge.
Occupational therapy services are provided in accordance with the plan of care and are case managed by qualified licensed Occupational Therapists (OT). Additionally, OT may assign qualified OTAs (Occupational Therapy Assistants) who service cases under the supervision of the OT and assist the OT in the implementation of the plan of care.
Speech Therapy services are provided in accordance with the Plan of care and are provided by a qualified licensed speech therapist. ST may develop rehabilitative plans of care, collaborate with the attending MD to obtain orders, provide education to patients and families and set achievable goals to discharge.
A certified nursing assistant (CNA) provides varying levels of care and support to physically and/or mentally disabled individuals who are unable to care for themselves. CNAs care for patients of all ages in hospitals, inpatient mental health facilities, and clinics.
Non-Medical Services
HHA services are ordered by the primary case manager (PCM) as appropriate to each case. The referral for HHA service is written and forwarded to the Scheduling department for assignment. HHAs assist with personal care and hygiene, assist with ambulation, transfers and exercise programs, provide basic nutritional support, provide light housekeeping tasks related to the patient.
An LNA provides basic nursing care which includes assistance with bathing, dressing, eating, mobility and toileting. They also take vital signs, obtain specimens, observe and report information, as well as many other tasks.
A RN is a clinician with a nursing degree who has passed a licensing exam.

Our fully-screened and certified personal care attendants are available to assist you with everyday tasks, such as eating, bathing, or light housework. We’re unique in that we only hire caregivers who exemplify compassion and empathy, and then match them with you based on personality and lifestyle, so you get a caregiver that’s right for you.
Companionship care services include:
- Adult and elderly home companion visits
- Assistance with writing letters and correspondence
- Games, crafts, and friendly conversation
- Assistance with organization
- Respite and family relief
- Sitter services
Housekeeping services include:
- Dusting
- Vaccuming
- Dish washing
- Laundry
- Meal planning and preparation
- Assistance with organization
If a client’s primary caregiver needs to take a temporary break, our respite care program provides a relief and assistance to client according to the needs of the client or the plan of care established by the client’s physician.
Alzheimer’s and dementia care includes:
- Promote optimal health with meal preparation
- Assistance with hygiene such as bathing, dressing and grooming
- Transportation for errands, shopping and doctor’s appointments
- Help with household chores like cleaning, laundry and dishes
- Reminders for medications and appointments
- Full time monitoring for maximum safety and comfort

Schedule a caregiver for as little as 2 consecutive hours or up to full-time support services 24 hours a day! (weekdays, weekends, holidays, etc.)
